People typically do not use gloves when playing in a private residence or hotel room with a single partner or small group (less than five people). People may opt for gloves in larger groups, with depth play, or in venues with inadequate washing facilities.

Do not assume your partner follows these practices. Specify—before you meet—your preferences or your present needs that alter your preferences. For example, if you generally play without gloves but have a cut on your hand and need protection, you should inform your partner before scheduling a session.

While BYOG (Bring Your Own Gloves) sounds reasonable, guys that only play S2S may not keep gloves in stock. The Provisions Matrix indicates that the responsibility falls upon the individual who requests gloves. This means a bottom who requires gloves will need to have several pairs of each size on hand, regardless of hosting or travelling for the session.

"It's the bottom's choice" is a common phrase you will hear when discussing glove etiquette. This would be true if gloves were always optional for both parties, but they are not. Sometimes, tops must wear gloves.

Neither party should be forced or harassed to fist in a manner with which they are not comfortable. Bottom's Choice should be replaced by Mutual Consent.

Standard Practices

Larry Shockey, the originator of FISTFEST, shares this piece of advice in his book, the Sacred Art of Fisting:

Most venues are not built with post-fisting cleanup in mind. Sinks may be too shallow to wash. Soap may be unavailable. Faucets may have restricted water output.

Outdoor venues or semi-public areas usually lack proper cleanup facilities.

Bring and wear gloves when playing at these venues—especially if you are fisting more than one person.

I coordinated my first trip to Amsterdam to coincide with the Dirty Dicks monthly fisting party at the Amsterdam Eagle. I packed a small gear bag with lube, poppers, and gloves and headed to the venue to play.

The event did not disappoint. I bottomed for four different guys and topped six. Unfortunately, I only brought four pair of gloves. This led to a little predicament with Rene, my last bottom.

Etiquette Explained

If you are wearing gloves for hygienic reasons, you still need to wash your hands and lower forearms between partners.

Most gloves cover about about 2.5 centimeters (1 inch) up the wrist. The anal canal is that same length, if not a bit longer. This means that as a top playing with shallow bottoms, your wrist and lower forearm is still likely be exposed to lube. Any deeper play guarantees exposure unless you are wearing opera length gloves.

Washing the hands and forearms prevents transfering microorganisms into your bottom.

While you may not need to scrub up to the shoulder, basic fisting typically exposes the lower forearm (above the glove line) to lube.

Erik and I had just wrapped up an intense fisting session where his small arms had slipped in past the elbow—a first for him. He had used both the right and left arms with depth play, but per my request, primarily played with the left arm.

After I said "OK, I'm done!", I asked him to hand me the paper towel roll.

"I find that if we don't squeegee the lube off now, your sink time will be 5-10 minutes clearing lube off your arm. And while it doesn't matter here with just the two of us and my deep sink, at events, you may not have the luxury of good washing facilities to clean up between bottoms. It encompasses a variety of factors, including the pathogen's ability to enter and survive in the host, evade the host's immune defenses, and cause damage to host tissues. Microorganisms vary widely in their degree of pathogenicity. Some pathogens are highly virulent, meaning they are very effective at causing disease, while others may be less virulent and cause milder infections. Chlaymdia (Oral)

Oral (or pharyngeal) chlamydia is a disease caused by Chlamydia trachomatis bacteria. If you have this infection, you can spread it to others unknowingly by eating ass or sucking cock.

In a majority of men (including fisters), oral chlamydia is asymtomatic, meaning you have no observed symptoms. To prevent re-infection from your regulars, it important to get tested if you are notified that you are exposed. It is also important to notify your playmates if you have a positive diagnosis or were treated for exposure.

The CDC suggests that gay men that are sexually active with multiple partners test for orophyarngeal chlamydia every three to six months.[1]

Bisexual men that have multiple partners and eat pussy should test every three months because oral to vaginal transmission can occur. Your female partners can become infertile due to undiagnosed chlamydia infections.

Do not rely on your health care provider to suggest oral swabbing, especially if your provider is a rural provider (in metropolitan areas less than 250,000) or if he does not specialize in gay men's health or infectious disease. You must ask for this test specifically.

 
Frequently Asked Questions
Can I get oral chlamydia from fisting?

It is possible, but unlikely. Chlamydia can survive outside the body for about 2-3 hours in humid environments. For you to contract oral chlamydia, the inside of your mouth or throat must be in physical contact with an object that was exposed to chlamydia less than three hours prior. These objects might include: a toy, specifically a lubed-up toy, a dick, fingers, or a hand. Most websites only mention washing toys between partners to prevent chlamydia spread; it is unlikely that any of those websites consider activities that occur while fisting in their advice.

Can I get oral chlamydia from spit and spit play?

It is near impossible to transmit chlamydia via spit. The enzymes in spit almost always prevent transmission from the infected to uninfected.

Other bodily fluids, such as semen, pre-cum, or vaginal secretions can transmit chlamydia to the oral cavity. Tainted lube from rectal or genital chlamydia may also carry and transmit the bacteria from the infected area to the uninfected oral cavitiy.

Membrane to membrane exposure is the most viable route (mouth to hole, mouth to rosebud, mouth to prolapse, mouth to piss slit).

I sucked off my bottom while fisting him. It depends. There are a lot of factors here. Since you are thrice-or-more removed from the original source, and since all those partners all played within potential transmissibility windows, it's possible to be infected.

You have three options:

  1. Quarantine for about 2-3 weeks and continuously inquire if anybody in the chain, other than the confirmed case, has since tested positive. The two-week quarantine is required because testing results prior to two weeks can report false negatives.
  2. Remain sexually active, but monitor for symptoms and test in two weeks. Keep a sex log with all your partners and their contact details for that time period. Do not have anonymous sex. Should you test positive in the next month, notify partners for them to get treated. Always remember to discuss STI status with your partners before playing.
  3. Get treated now. Sometimes your provider requires that you had direct contact with an infected person. You may have to distort the truth a little to get treatment now. Your quarantine will be much less if you take this approach.
Can I get oral chlamydia from fist nectar or from swallowing my partner's piss?

Yes to fist nectar. Fist nectar contains bodily fluids that will pass on chlamydia (usually precum and semen).

As far as urine is concerned, the data is unclear. Most websites indicate urine does not transmit chlamydia (so you won't catch it from a toilet seat); however, these same websites mention that contaminated piss spilled on somebody's hand (poor bathroom hygiene), or recent piss on a surface that comes in contact with your oral cavity can spread chlamydia to your mouth. As mentioned previously, author's of these websites aren't taking fisting nor other kink-related behaviors into consideration.

Will oral chlamydia clear up on its own?

Symptoms may disappear, but it will not clear up on its own without antibiotic treatment.

Why be vigilant?

It's human nature to deny the possibility of infection. Common excuses you may hear, or even say yourself include:

  • He didn't cum in my mouth.
  • I didn't suck his dick that long or eat his ass that much.
  • I swallowed his cum immediately so there was no time to infect my throat.
  • My sore throat is because his dick was so huge it bruised the fuck out of my mouth and throat.
  • My sore throat is due to cold—the weather or all that yelling I did at the football game.
  • I'm feeling fatigued and worn down because I drank a My body hates me because I just drank too much. I just stayed out too long or I'm super stressed. I don't show any symptoms even though my partner told me he has it. 7. HOW LONG DOES IT TAKE FOR ORAL CHLAMYDIA TO SHOW UP? Typically, it takes one to three weeks. If you are told by a partner that he has chlamydia, and you sucked his dick, ate his ass, or vice versa—don't sit around and wait for symptoms to show up. They may not (you might just be asymptomatic). Just schedule a test or treat immediately. 8. WHY DOES MY HEALTH CARE PROVIDER HAVE TO KNOW HOW MANY PARTNERS I'VE HAD AND WHAT TYPE OF SEX WE HAD? Your doctor is trying to identify high risk behavior in order to: provide education, eliminate other potential causes of your symptoms, assess the need to test for related conditions, and complete the necessary items for billing (so that the claim isn't denied, and the costs are passed down to you). Your provider is also part of the public health system and has an ethical duty to contribute to the reduction/eradication of STIs. 9. WHEN SHOULD I TEST? Typically, it requires a two-week period for chlamydia to show up on tests. Testing before that time may give false negatives. In the USA, if you have had a partner with confirmed Chlamydia, they usually treat you without waiting for test results. Additionally, if you are sexually active with multiple partners you should test every three months for oral chlamydia. 10. CAN I TEST AT HOME WITHOUT A DOCTOR? Yes. In the USA, you can buy tests, in other countries, at home tests may be available online or from the public health departments. Remember that it takes about two weeks from exposure for accurate testing results. 11. CAN I SPREAD ORAL CHLAMYDIA IF I'M ASYMPTOMATIC OR IN THE INCUBATION PERIOD? Yes and yes; however, it is highly unlikely that you will have spread it to others at the orgy where you contracted it. It needs some time to get established in your body first. 12. WHY DOES ANY OF THIS MATTER? First, long term infection with any disease increases the probability of other poor health outcomes. Second, the fisting ‘swimming pool’ is shallow. That is to say, we all play with each other with very few degrees of separation. One person in the pool that neglects his health and refuses testing and treatment can spread this disease to an entire community, and it can happen quickly. The CDC suggests that gay men that are sexually active with multiple partners test for oropharyngeal chlamydia every three to six months. Bisexual men have additional responsibilities. If they eat pussy, as in a chromosomal vagina, they should test every three months because oral to vaginal transmission can occur. Giving your female partners chlamydia can result in their infertility. 13. WHAT IF MY DOCTOR DOESN’T OFFER THE SWAB? Do not rely on your health care provider to suggest oral swabbing, especially if: your provider’s practice is in a metropolitan area less than 250,000, in the outer suburbia, in rural areas, or your provider does not specialize in gay men's health or infectious disease, or your provider isn’t exposed to many gay patients, or your provider thinks you are straight, or your provider doesn’t accurately know about your sex life and number of partners, or your provider is older than 65, or you have a horrible provider from the NHS, or your provider comes from a different culture that isn't aware of gay men's health issues (Mormons, for example). You should suggest the swab. Most STI clinics will have swabs available upon request. Most providers can send you out to labs that have the swabs available for testing. Do not assume that your provider will test these things with your annual physical. Request them.
    Can I get oral chlamydia from fisting alone?

    If you are a pervert with a big mouth (like me) and can take a fist in your mouth (like I long to do), especially one that has been in an ass just a few minutes previous, then maybe. Chlamydia can be transmitted via toys—so theoretically, a fist in your mouth could do the same.

    If your fist session did not involve sucking his dick or eating his ass or swallowing his load or spitting out his load because it tasted awful, then you are safe.

    Can I get oral chlamydia from spit play?

    No. Enzymes in saliva prevent transmission. It's usually semen, pre-cum or direct contact of mucosal membranes (such as tongue to hole [ass or piss slit]).

    Can I get oral chlamydia from fist nectar or from swallowing his/her piss?

    Yes to fist nectar and probably no to straight up piss. Fist nectar contains bodily fluids that will pass on Chlamydia (usually piss, precum, semen). The risk for transmitting oral chlamydia from just urine during piss play while fisting is very very low, but not completely improbable.

    Will oral chlamydia clear up on its own?

    Symptoms may disappear, but it will not clear up on its own without treatment.

    What excuses may I conjure up to convince myself that I don't have chlamydia and don't need to test?

    You may try one of these:

    • He didn't cum in my mouth.

    How long does it take to show up?

    Typical 1 to 3 weeks. If you are told by a partner that he has chlamydia and you sucked his dick, ate his ass, or vice versa, don't sit around and wait for symptoms to show up. They may not. Just schedule a test.

    Why does my health care provider have to know how many partners I've had and what type of sex we had?

This infection can be asymptomatic and is easily transmitted, so if you are notified by a parter of a possible exposure, get tested or refrain from sexual contact for three weeks.
